GENERAL RELIEF

Effective December 1, 2015, all General Relief Housing Subsidy and Case Management Project (GRHSCMP) Housing staff must utilize the PA 6145, GRHSCMP Checklist (11/2015) to determine if a General Relief (GR) Housing applicant/ participant is a “heavy user” of County services under the Unemployable target population.
The Housing Case Managers (HCMs) will determine if the applicant/participant is a “heavy user” based on history in California Statewide Automated Welfare System (CalSAWS) and/or the applicant’s/participant’s self-declaration of receiving any County services from the Department of Health Services (DHS), Department of Mental Health (DMH), or
Los Angeles Sheriff’s Department (LASD).
If the applicant/participant is determined to be a “heavy user,” priority will be given to the applicant’s/participant’s housing subsidy request.
The Help Through Enterprise Linkage Project (H.ELP) System sunsetted November 30, 2015, as the contract with SAS Institute Incorporated expired. The H.ELP System was previously used to review an applicant’s/participant’s service records with other County departments to determine if a GR Housing applicant/participant was a “heavy user” of County services.
Heavy User is defined as a GR applicant/participant who has received the following services:
Extensive use of services from DHS
Two or more admissions for inpatient Hospitalization and/or Emergency-based services
Extensive use of services from DMH
Had been incarcerated in a Los Angeles County Jail for over 60 days
Used medical or mental health services while incarcerated within the last 12 months
Had been identified based on the participant’s statements as recorded on the PA 6142, GRHSCMP Checklist-Heavy User Determination
Any of the above services from DMH, DHS and/or LASD
